Who this template is for

Campaign Pledge Card is built for small teams who need a reliable intake process who need to collect the same complete information from everyone.

  • Small teams who need a reliable intake process working in fundraising & alumni.
  • Teams who need to collect the same complete information from everyone without writing code or paying for a custom build.
  • Anyone replacing chasing missing details over email with one structured record per enquiry.
  • Respondents are the people you send it to — the form asks them 21 questions across 3 screens.
  • Mobile-heavy audiences, thanks to the one-question-per-screen card layout.

Why this form is useful

It removes the cost of chasing missing details over email and turns each submission into a record your team can act on immediately.

  • 9 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It collects a verified email address so replies and confirmations land.
  • It captures a phone number for urgent follow-up.
  • It records a signature you can keep as proof of agreement.
  • It pins the request to a specific date.
  • It captures a full postal address.
  • It captures a numeric value you can filter and sort on.
  • Conditional logic hides 6 questions until they are relevant — shorter forms convert better than long ones.

  • Features advanced show logic: if “Payment method” is Card or bank transfer, the form dynamically exposes “Preferred billing day of the month”. Otherwise that question never appears.
  • Features advanced show logic: if “Payment method” is Stock or donor-advised fund, the form dynamically exposes “Advisor or broker details”. Otherwise that question never appears.
  • Features conditional validation: if “Will your employer match this gift?” is Yes, “Employer name” becomes mandatory before the form can be sent.
  • Features conditional hiding: if “Should this gift be anonymous?” is Yes, “How should your name appear in the donor listing?” is removed from the form and is never submitted.
  • All 6 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

A campaign pledge is a promise spread across time, so the card has to record the total, the schedule and the recognition in the same breath. This one is written for capital and multi-year appeals rather than one-off giving. The donor picks the campaign, states the total amount pledged, and chooses how it will be fulfilled: in full now, annually across two to five years, quarterly or monthly. Choosing instalments reveals the start date and the per-instalment amount so nobody has to work it out later. Payment method branches too — a card or bank transfer asks for the preferred billing day, a cheque asks where it will be posted from, and a stock or donor-advised-fund gift opens a note field for the advisor's details. Recognition is asked properly instead of assumed: how the name should appear in the donor listing, whether the gift should be anonymous, and whether the donor wants the pledge counted in a named giving level. A matching-gift branch captures the employer when one applies, and the card closes with a signature and date so the finance team has a document it can actually book.

How the form changes as it's filled in

This template ships with 6 conditional rules, grouped into 5 behaviours driven by 4 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “How would you like to fulfil this pledge?

  • Reveals questions

If “How would you like to fulfil this pledge?” is not In full now, the form reveals “First instalment date” and “Amount per instalment”.

Driven by “Payment method

  • Reveals questions

If “Payment method” is Card or bank transfer, the form reveals “Preferred billing day of the month”.

  • Reveals questions

If “Payment method” is Stock or donor-advised fund, the form reveals “Advisor or broker details”.

Driven by “Will your employer match this gift?

  • Makes answers required

If “Will your employer match this gift?” is Yes, “Employer name” becomes required before the form can be submitted.

Driven by “Should this gift be anonymous?

  • Hides questions

If “Should this gift be anonymous?” is Yes, the form hides “How should your name appear in the donor listing?”, so nobody answers questions that don't apply.

Set it up step by step

  1. 1

Name the campaign in the link

Pre-fill the campaign field from the URL so the same card can be used at every event without a volunteer retyping it.

  2. 2

Print it and keep it online

The card layout prints cleanly for gala tables; the same link works on a phone for follow-up conversations the next morning.

  3. 3

    Set the instalment reminders

Export pledges with their first instalment date and load them into your finance calendar the week the campaign closes.

  4. 4

    Honour the anonymity flag

Filter the donor listing on the anonymous answer before anything goes to print.

Mistakes to avoid

  • Recording a total with no schedule, so the pledge cannot be tracked against payments.
  • Publishing a donor listing without checking the anonymity answer.
  • Ignoring the matching-gift question, which is the cheapest uplift in most campaigns.

What it pairs with

Most teams don't run this form on its own. These are the forms and systems it sits next to.

What to do with the responses

Book the pledge in your fundraising ledger, send a written confirmation showing the schedule, and diarise the first instalment reminder.

Works in both layouts

These pages cover the same subject ground as the campaign pledge card, matched on the words people actually search for.

Privacy & compliance

This form takes money. Card details are handled by the payment provider and never stored in your response data — you keep the order details, not the card number.

Frequently asked questions

How is this different from the donation pledge card?

The Donation Pledge Card is for single or recurring gifts. This one is written for campaigns: multi-year schedules, recognition levels and a signature the finance team can file.

Can we collect the first payment straight away?

Yes — add a payment field to the second page and connect your payment provider; the schedule fields still record the rest of the commitment.

Is a signed pledge legally binding?

That depends on your jurisdiction and how the pledge is worded. Treat it as a record of intent and take advice before relying on it as a receivable.

Can volunteers fill it in on someone's behalf?

Yes. Add a short "recorded by" field at the end so you know which volunteer took the conversation.

Is the Campaign Pledge Card template free to use?

Yes. You can preview and test Campaign Pledge Card on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.

What does the Campaign Pledge Card template ask for?

It asks 21 questions across 3 pages, 9 of which are required. Every question is listed in full further down this page, and each one can be renamed, reordered, made optional or deleted after you copy the template.

Can I edit the Campaign Pledge Card form after copying it?

Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the card and classic layouts, restyle it to match your brand, and set who gets notified on each submission.

How does the conditional logic in this fundraising & alumni form work?

Where do responses to the Campaign Pledge Card form go?

Can I embed the Campaign Pledge Card form on my own website?

Yes. Publish the form and paste the embed snippet into any page or share the direct link. The embedded form resizes to fit its container and loads no advertising or marketing trackers inside the iframe.

Is the signature on this form legally usable?

The template captures a drawn signature and stores it with the submission, along with the timestamp, so you keep evidence of who agreed and when. Whether that is sufficient depends on your jurisdiction and the agreement itself.
