This form is deliberately short and deliberately granular: name, email, and then a separate permission for each channel you might use — email, post, phone and text — because a single combined tick box collapses the moment a supporter objects to one of them. Providing a phone number opens the phone and text permissions rather than assuming them, and providing a postal address opens the post permission, so no channel is ever recorded as allowed on the strength of the data existing. Interests are captured as a short list of the causes or appeals you run, which is what lets you send the flood appeal to people who care about floods rather than to everybody. Frequency is offered as a genuine choice including a once-a-year option, because giving supporters a lower setting keeps far more of them on the list than the alternative of unsubscribing. The consent statement records the date and the wording shown, and the confirmation page explains exactly how to change or withdraw permission later.
