Compliance checklist
- Payment items priced — passingEvery payment question charges a real amount, so a submission always records a payment.
- Conditional logic resolves — passingEvery rule points at a question or page that exists in this template.
- Medical disclaimer present — passingThis form does not provide medical advice, diagnosis or treatment. Information you share here is not a substitute for care from a qualified clinician. In an emergency, contact your local emergency service immediately.
- Question labels unique — passingLogic and exports address questions by label, so duplicates are ambiguous.

Who this template is for
Coaching Progress Check-In is built for clinics, coaches and therapists who need to collect intake detail and consent before the first session.
- Clinics, coaches and therapists working in fitness, wellness & coaching.
- Teams who need to collect intake detail and consent before the first session without writing code or paying for a custom build.
- Anyone replacing paperwork eating the start of every appointment with one structured record per enquiry.
- Respondents are clients and patients — the form asks them 10 questions across 1 screen.
- Mobile-heavy audiences, thanks to the one-question-per-screen card layout.
Why this form is useful
It removes the cost of paperwork eating the start of every appointment and turns each submission into a record your team can act on immediately.
- 4 questions are required, so submissions arrive complete instead of needing a follow-up email.
- It pins the request to a specific date.
- It scores satisfaction on a consistent scale.
- It measures sentiment on a numeric scale.
- Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
- Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint
Exactly how this form behaves as it is answered — 3 conditional rules ship with it.
- Features conditional validation: if “Progress rating” is less than 4, “What got in the way?” becomes mandatory before the form can be sent.
- Features advanced show logic: if “Progress rating” is less than 4, the form dynamically exposes “Bring our next session forward?”. Otherwise that question never appears.
- Features conditional validation: if “Need support before next session?” is Yes, “What support do you need?” becomes mandatory before the form can be sent.
- All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.
About this template
Most coaching progress is lost in the gap between sessions, and this weekly check-in is the cheapest way to close it. It takes two minutes: what was completed, what slipped, and two numbers — a progress rating and a confidence score — that become a trend line over a programme. Rating progress at three or below requires a short explanation of what got in the way and offers an earlier session, which is how you catch a client drifting before they quietly cancel. Confidence tracked separately from progress is deliberate: clients often do the work while losing belief, and that pattern only shows when the two are asked apart. The support question surfaces requests people would not raise unprompted, and the optional session-focus answer means you arrive at the next call already knowing where to start rather than spending ten minutes finding out.

How the form changes as it's filled in
This template ships with 3 conditional rules, grouped into 3 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.
Driven by “Progress rating”
- Makes answers required
If “Progress rating” is less than 4, “What got in the way?” becomes required before the form can be submitted.
- Reveals questions
If “Progress rating” is less than 4, the form reveals “Bring our next session forward?”.
Driven by “Need support before next session?”
- Makes answers required
If “Need support before next session?” is Yes, “What support do you need?” becomes required before the form can be submitted.
Set it up step by step
- 1
Send it the same day each week
A fixed rhythm turns the check-in into a habit rather than an interruption.
- 2
Chart both numbers
Progress and confidence diverging is the earliest warning sign you will get.
- 3
Open the session with their answer
Starting from the stated focus makes the first five minutes productive.
- 4
Act on the low scores
Offering an earlier session after a bad week is what prevents a quiet cancellation.
Mistakes to avoid
- Asking only for a rating, with no space to explain it.
- Collecting check-ins and never referring to them in session, which teaches clients to stop bothering.
- Making it long — anything over ten questions weekly gets abandoned.
What it pairs with
Most teams don't run this form on its own. These are the forms and systems it sits next to.
What to do with the responses
Review before each session, track the two scores over the programme, and respond the same day to any low rating or support request.

Frequently asked questions
How often should clients check in?
Weekly for most programmes, fortnightly for monthly coaching. Match it to the session frequency in the intake.
What if a client stops completing it?
That is itself the signal. Ask about it directly in the next session rather than sending another reminder.
Should I share the trend with the client?
Yes — showing the progress and confidence lines is often more motivating than any conversation about them.
Where do the goals come from?
The Coaching Intake Form, which captures the primary goal in the client's own words.
