About this template
A home assessment is only useful if it records the home as well as the person. This three-page form is written to be completed on a visit. The first page covers the person and the people around them: who they live with, who provides informal support, next of kin, the services already involved and who holds a key or knows the entry arrangements. The second covers function honestly rather than optimistically — walking indoors and outdoors, stairs, transfers, washing and dressing, using the toilet, preparing food and eating, continence, memory and communication — with each answer graded by how much help is actually needed. The third page is the property: access and steps, lighting, floor coverings and trip hazards, bathroom layout, heating, working smoke alarms, stair rails and any equipment already in place, plus whether the home is warm and food is available. It closes with what the visiting clinician judges is needed and what has been agreed with the person. The record is the visit; the plan follows from it.
