  • Legal text not required — passingThis template collects neither health nor regulated financial information.
  • Question labels unique — passingLogic and exports address questions by label, so duplicates are ambiguous.

Who this template is for

Shipment Booking Form is built for carriers, couriers and operations teams who need to record consignment detail and proof at each handover.

  • Carriers, couriers and operations teams working in logistics & field ops.
  • Teams who need to record consignment detail and proof at each handover without writing code or paying for a custom build.
  • Anyone replacing disputes with no record of what was collected or delivered with one structured record per enquiry.
  • Respondents are shippers, drivers and warehouse staff — the form asks them 15 questions across 2 screens.
  • Mobile-heavy audiences, thanks to the one-question-per-screen card layout.

Why this form is useful

It removes the cost of disputes with no record of what was collected or delivered and turns each submission into a record your team can act on immediately.

  • 8 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It captures a phone number for urgent follow-up.
  • It pins the request to a specific date.
  • Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
  • Splitting the form across 2 pages keeps each screen short and shows respondents how much is left.
  • Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 3 conditional rules ship with it.

  • Features advanced show logic: if “Loading method” is Hand load, the form dynamically exposes “Extra labour needed?”. Otherwise that question never appears.
  • Features advanced show logic: if “Confirm weight and dimensions” is No, the form dynamically exposes “Corrected weight and dimensions”. Otherwise that question never appears.
  • Features conditional validation: if “Confirm weight and dimensions” is No, “Corrected weight and dimensions” becomes mandatory before the form can be sent.
  • All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

Booking is where a quote meets a real site, and the details that were irrelevant to pricing suddenly decide whether the collection succeeds. This form asks for them explicitly. The quote reference carries the agreed rate forward so nothing is re-quoted by accident. Pickup date and time window set the driver's plan, and the loading method question — dock, forklift, tail-lift or by hand — is the one most often assumed and most often wrong; choosing hand loading offers an extra-labour question before the driver arrives alone. Site constraints and driver instructions capture the low bridge, the narrow lane and the security gate that would otherwise turn into an aborted collection charge. The final confirmation asks the shipper to stand behind the weight and dimensions from the quote, and answering no requires corrected figures, which is far cheaper than discovering the discrepancy at the weighbridge. Terms acceptance completes a booking record both sides can rely on.

How the form changes as it's filled in

This template ships with 3 conditional rules, grouped into 3 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “Loading method

  • Reveals questions

If “Loading method” is Hand load, the form reveals “Extra labour needed?”.

Driven by “Confirm weight and dimensions

  • Reveals questions

If “Confirm weight and dimensions” is No, the form reveals “Corrected weight and dimensions”.

  • Makes answers required

If “Confirm weight and dimensions” is No, “Corrected weight and dimensions” becomes required before the form can be submitted.

Set it up step by step

  1. 1

    Book against the quote reference

It is the only reliable way to make sure the load moves at the rate that was agreed.

  2. 2

    Take the site constraints seriously

Most aborted collections are caused by something the shipper knew and was never asked.

  3. 3

    Confirm the window in writing

Reply with the booked window and the driver's contact so the site can plan around it.

  4. 4

    Re-check corrected dimensions

If the shipper amends weight or dimensions here, requote before dispatch rather than after.

Mistakes to avoid

  • Assuming a forklift is available because the last collection from that site had one.
  • Booking without a contact phone, so a driver held at a gate has nobody to call.
  • Accepting unchanged dimensions from the quote when the load has since grown.

What it pairs with

Most teams don't run this form on its own. These are the forms and systems it sits next to.

What to do with the responses

Confirm the booking window in writing, dispatch the load with the site notes attached, and close it with a proof of delivery.

Works in both layouts

These pages cover the same subject ground as the shipment booking form, matched on the words people actually search for.

Frequently asked questions

Should the shipper or the broker complete this?

The shipper, because the site constraints and contact details live with them. Brokers usually send the link on acceptance.

What if dimensions change after booking?

Ask for a fresh submission rather than an email amendment, so the booked record matches what will be loaded.

Does accepting terms here count?

It records acceptance with a timestamp; make sure your terms of carriage are linked from the form itself.

What closes the job?

The Proof of Delivery Form, completed by the driver at the delivery point.
