1. Car rental booking

Ask for pickup and drop-off location and date, vehicle category, and driver age โ€” age brackets affect eligibility and pricing for most rental businesses, so it belongs on the form, not discovered at the counter. Add-ons (child seat, additional driver, insurance tier) work best as clear optional selections shown with their price attached, rather than upsold verbally at handover, which builds distrust when the final bill is higher than expected.

2. Auto repair quote

The single highest-value field here is a description of the problem paired with a video or photo upload, plus vehicle make, model, year and mileage. Ask whether the vehicle is currently driveable โ€” it changes whether the quote assumes a drop-off or a pickup. 3. Lost and found claim

This form exists to prevent fraudulent claims as much as to help genuine ones. Ask the claimant to describe the item in detail before showing them any description of what was found โ€” a claimant who can name a distinguishing mark or contents is far more likely to be the real owner. Capture where and roughly when the item was lost, and always require a callback or collection method rather than shipping items out automatically.

4. Pet grooming appointment

Ask for breed, size and coat type, since these determine appointment length and price more than almost anything else. A behaviour question โ€” "is your pet nervous around clippers or other animals?" โ€” lets groomers schedule appropriately and avoid double-booking a nervous large dog next to a boisterous puppy. Vaccination status is worth a required field if your business (or local rules) requires it for boarding-adjacent services.

5. Catering menu customisation

Dietary requirements deserve their own clearly labelled, required section โ€” allergies especially should never be an afterthought in a general notes box. Ask for guest count, event date and service style (buffet, plated, drop-off) as separate fields, since each changes staffing and pricing independently. Give a realistic lead-time notice on the form itself ("orders need 5 working days' notice") so people don't submit a same-day request expecting a same-day answer.

Claims, deposits and getting the money conversation right

Any form tied to a deposit, a security hold or a claim should say plainly, before submission, what happens with payment โ€” when a card is charged, what a deposit covers, and how a refund or dispute is handled. For lost-and-found and repair claims specifically, keep a clear internal log of who verified what, since these are the forms most likely to be queried after the fact.
