About this template
Boards are usually recruited through a conversation and a nod, which is how organisations end up with a skills gap nobody noticed and a conflict of interest nobody declared. This application makes the process deliberate. The first page covers the candidate and their motivation: why this organisation, what they understand its work to be, and whether they have been a trustee or board member before. The second page is the skills audit that a board actually needs — finance, legal, fundraising, digital, HR, safeguarding, service-user experience, sector knowledge — with space to describe where each was used, plus governance experience, committee interest and any chair or treasurer ambition. The third page is the part that protects the organisation: current and previous directorships and trusteeships, employment that could create a conflict, family or business connections to staff or suppliers, an explicit conflict-of-interest declaration, and confirmation that the candidate is not disqualified from serving under the rules that apply to your organisation. Time commitment is asked for honestly — meetings per year, evening availability, reading time — because most trustee resignations trace back to a commitment that was never described. References and consent to any required checks close the form.

Set it up step by step
- 1
Recruit against a skills gap
Publishing the skills you need and asking candidates to map onto them is what changes a board's composition.
- 2
Ask for hours a month
Most trustee resignations come from a commitment nobody described honestly at the start.
- 3
Collect interests at application
A conflicts declaration gathered up front is far easier than reconstructing one before a contested decision.
- 4
Ask about eligibility plainly
Rules on who may serve differ by country and constitution, so ask the question and let your governance check the answer.
Mistakes to avoid
- Appointing through informal networks, which quietly reproduces the board you already have.
- Never asking about other directorships, then discovering an overlap during a funding decision.
- Describing the role as "a few meetings a year" when it involves reading packs and joining a committee.
What it pairs with
Most teams don't run this form on its own. These are the forms and systems it sits next to.
What to do with the responses
Shortlist against the skills gap, interview, take up references, complete any required checks, and record the appointment and declared interests in your register.

Frequently asked questions
Is this a legal eligibility check?
No. It asks the candidate to confirm eligibility and consent to checks. What is required depends on your constitution and local law.
Why ask about other trusteeships?
Both for capacity and for conflicts. A candidate on four boards may not have the time, or may sit on a funder's board.
Should references be taken before interview?
The candidate chooses on the form, which avoids approaching a current employer before anyone has met.
Can it be used for advisory committees?
Yes, though the eligibility and conflicts questions matter most for a governing board.
Is the Board Member Application template free to use?
Yes. You can preview and test Board Member Application on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.

Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the classic layout, restyle it to match your brand, and set who gets notified on each submission.

Yes. Publish the form and paste the embed snippet into any page or share the direct link. The embedded form resizes to fit its container and loads no advertising or marketing trackers inside the iframe.
Is the signature on this form legally usable?
The template captures a drawn signature and stores it with the submission, along with the timestamp, so you keep evidence of who agreed and when. Whether that is sufficient depends on your jurisdiction and the agreement itself.
