About this template
When an insurer asks a witness for an account, the account has to attach to a specific claim or it cannot be used. That is what separates this form from a general statement. It opens with the claim reference, the policy number and the name of the insured party, so the submission files itself against the right claim, and it asks how the witness came to be involved and whether they are connected to the insured — a relationship an adjuster needs to know about up front rather than discover later. The middle section is the account: the date and time, where the witness was, what they saw or heard in sequence, the condition of any vehicle or property they noticed, and what happened immediately afterwards. Two questions ask whether they have been contacted by anyone else about the claim and whether they have any photographs or footage. It closes with a signed confirmation that the account is their own. The form records evidence; the assessment of the claim remains the insurer's, and no answer here decides it.
