  • Payment items priced — passingEvery payment question charges a real amount, so a submission always records a payment.
  • Conditional logic resolves — passingEvery rule points at a question or page that exists in this template.
  • Financial disclaimer present — passingThis form does not provide financial, tax or legal advice. Any figures shown are indicative only and are not an offer of credit or a guarantee of terms. Please seek advice from a regulated adviser before making a decision.
  • Question labels unique — passingLogic and exports address questions by label, so duplicates are ambiguous.

Who this template is for

Insurance Certificate is built for small teams who need a reliable intake process who need to collect the same complete information from everyone.

  • Small teams who need a reliable intake process working in insurance.
  • Teams who need to collect the same complete information from everyone without writing code or paying for a custom build.
  • Anyone replacing chasing missing details over email with one structured record per enquiry.
  • Respondents are the people you send it to — the form asks them 14 questions across 2 screens.
  • Mobile-heavy audiences, thanks to the one-question-per-screen card layout.

Why this form is useful

It removes the cost of chasing missing details over email and turns each submission into a record your team can act on immediately.

  • 9 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It collects a verified email address so replies and confirmations land.
  • It accepts a document or photo upload as evidence.
  • It pins the request to a specific date.
  • It captures a full postal address.
  • Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
  • Splitting the form across 2 pages keeps each screen short and shows respondents how much is left.
  • Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 3 conditional rules ship with it.

  • Features advanced show logic: if “Are There Special Certificate Requirements?” is Yes, the form dynamically exposes “Requirements Attachment”. Otherwise that question never appears.
  • Features advanced show logic: if “Are There Special Certificate Requirements?” is Yes, the form dynamically exposes “Requested Wording or Requirements”. Otherwise that question never appears.
  • Features advanced show logic: if “Send the Certificate To” is Me and the certificate holder, the form dynamically exposes “Certificate Holder Email”. Otherwise that question never appears.
  • All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

A certificate request should give the servicing team enough information to identify the insured business, name the certificate holder correctly, and understand where the document will be used. Keep those details separate: the business requesting proof of insurance is not necessarily the organization receiving it. Ask requesters to copy the certificate holder’s legal name and mailing address from the contract or onboarding instructions. Use the project field for a job number, site, lease, or event reference. A policy reference helps with matching, but leaving it optional prevents a missing number from blocking an otherwise useful request. For requests with special requirements, reveal an upload and a wording field. Encourage people to attach only the relevant insurance requirements rather than an entire contract. Treat uploaded language as material for review, not instructions that can automatically be accepted or added to a certificate. Route submissions to the team responsible for issuing certificates, with the needed-by date visible in its queue. Acknowledge receipt separately from confirming delivery. Before sending the document, check the holder details, recipient email, and any unresolved requirements. Submission alone should never be presented as confirmation of coverage or approval of requested wording.

How the form changes as it's filled in

This template ships with 3 conditional rules, grouped into 2 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “Are There Special Certificate Requirements?

  • Reveals questions

If “Are There Special Certificate Requirements?” is Yes, the form reveals “Requirements Attachment” and “Requested Wording or Requirements”.

Driven by “Send the Certificate To

  • Reveals questions

If “Send the Certificate To” is Me and the certificate holder, the form reveals “Certificate Holder Email”.

Set it up step by step

  1. 1

    Assign a certificate owner

Route submissions to the servicing team that can review and issue certificates. Include the insured business name and needed-by date in internal notifications.

  2. 2

    Keep holder details distinct

Preserve separate fields for the insured business and certificate holder. Ask reviewers to compare the holder name and address against the supplied instructions before issuance.

  3. 3

    Review exceptions before drafting

Check Yes and Not sure responses for follow-up. If a requester selects Yes without supplying an attachment or wording, ask for the requirements rather than guessing.

  4. 4

    Confirm delivery separately

Use the submission acknowledgment to confirm receipt only. Verify any certificate holder email and record the issued document’s delivery in your request tracker.

Mistakes to avoid

  • Entering the insured business in the certificate holder field instead of the organization requesting proof.
  • Treating requested wording or an uploaded sample as already approved.
  • Promising the requested delivery date before checking policy details and unresolved requirements.

What it pairs with

Most teams don't run this form on its own. These are the forms and systems it sits next to.

What to do with the responses

Acknowledge the request, match it to the insured account, and review any special requirements. Confirm recipient details before issuing and sending the certificate.

Works in both layouts

These pages cover the same subject ground as the insurance certificate, matched on the words people actually search for.

Frequently asked questions

Who should be listed as the certificate holder?

Enter the person or organization asking to receive proof of insurance, using the name and address in its instructions. Keep this separate from the insured business name.

Can this form request additional insured wording?

Yes. Select Yes for special requirements and attach or paste the instructions. The servicing team must review the request; submitting it does not approve wording or change coverage.

What if the request requires a policy change?

Use Policy Change Request to collect the change details separately. Keep the certificate request linked to that review so the team does not issue a document before the underlying request is resolved.

Can I submit without a policy number or recipient email?

Yes. The insured business name and your contact details let the team begin matching the request. If you request direct delivery but omit the holder’s email, the team will need to follow up before sending it to the holder.

Is the Insurance Certificate template free to use?

Yes. You can preview and test Insurance Certificate on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.

What does the Insurance Certificate template ask for?

It asks 14 questions across 2 pages, 9 of which are required. Every question is listed in full further down this page, and each one can be renamed, reordered, made optional or deleted after you copy the template.

Can I edit the Insurance Certificate form after copying it?

Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the classic and card layouts, restyle it to match your brand, and set who gets notified on each submission.

How does the conditional logic in this insurance form work?

3 conditional rules ship with the template: Features advanced show logic: if “Are There Special Certificate Requirements?” is Yes, the form dynamically exposes “Requirements Attachment”. Otherwise that question never appears. Features advanced show logic: if “Are There Special Certificate Requirements?” is Yes, the form dynamically exposes “Requested Wording or Requirements”. Otherwise that question never appears. Features advanced show logic: if “Send the Certificate To” is Me and the certificate holder, the form dynamically exposes “Certificate Holder Email”. Otherwise that question never appears. All rules are editable in the Logic tab.

Where do responses to the Insurance Certificate form go?

Submissions land in your workspace under Submissions, where you can search, filter, label and export them to CSV or PDF. You can also email a notification to your team on every submission and send the respondent an auto-reply.

Can I embed the Insurance Certificate form on my own website?

Yes. Publish the form and paste the embed snippet into any page or share the direct link. The embedded form resizes to fit its container and loads no advertising or marketing trackers inside the iframe.

Can respondents upload files?

Yes. This template includes a file upload question, so respondents can attach documents or photos with their answers. You can adjust the accepted file types and size limits on that question.
