This two-page version covers access first — how easy it was to get through, how long the wait for an appointment was, whether the appointment time offered suited — then the visit itself: reception, the waiting area, privacy, and whether the patient felt they had enough time with the clinician. Communication is asked separately from clinical care, because the two often diverge and only one of them is easy to fix. There is a question about whether the patient understood the plan they left with, and a recommendation question for a headline figure your board will ask for anyway. Two open boxes ask what works well and what should change, in that order, because starting with the negative shapes everything after it.
