About this template

During a vaccination session the record has to be written between patients, so it needs to be short and unambiguous. This form captures the dose as it is given: patient name and date of birth, the product administered, the batch number and expiry, the site and side used, the date, and the name of the person who gave it. Batch and expiry sit next to each other on purpose, because they are the fields most often left until later and hardest to reconstruct afterwards if a product is recalled. A dose-number question distinguishes a first dose from a subsequent one without a separate form. The last question notes anything observed in the minutes afterwards, with a short box that opens only when there is something to record, so a normal dose takes three taps. Consent belongs in its own form signed before the dose, and this record links to it rather than repeating it. Kept together, the session's submissions become a searchable log by product, batch and date instead of a stack of paper tally sheets.
