About this template

An encounter form — sometimes called a superbill or a charge slip — is the single sheet a clinic completes for each patient visit, recording what happened and what should be billed. It sits between the clinical note and the claim: shorter than the note, more structured than a memo, and the document a biller works from. This template follows that shape. Page one captures the visit itself: patient, date of birth, visit date, provider, visit type and the reason in the patient's own terms. Page two carries the clinical and billing content — diagnosis codes, services or procedure codes, prescriptions issued, whether a follow-up is needed, when, and the provider's signature. It differs from the New Patient Registration Form, which is completed once by the patient before care begins; this one is completed by the practice at every visit. Two rules protect the claim: a telehealth visit requires the patient's location during the appointment, and answering yes to follow-up requires a timeframe.
