About this template

A referral that arrives without context becomes a phone call, and phone calls become delays. This form structures the handover. Page one is the patient and the parties: name, date of birth, phone, the referring clinician and practice, and the specialty or clinic being referred to. Page two is the clinical content that determines triage: the reason for referral, relevant history and current medications, an urgency rating, any investigations attached as files, confirmation that the patient knows the referral is happening, and the referrer's signature. It differs from the Patient Encounter Form, which documents one consultation for the record; a referral is a request addressed to someone else, so it has to carry enough for a stranger to make a decision. Two rules enforce the parts that matter most: marking a referral urgent reveals a direct-contact note and requires a phone number the receiving clinic can ring today, and answering that the patient is unaware reveals a prompt to discuss it with them before sending, which prevents an appointment letter arriving as a surprise.
