Who this template is for

Forklift Inspection Checklist is built for small teams who need a reliable intake process who need to collect the same complete information from everyone.

  • Small teams who need a reliable intake process working in manufacturing & industrial.
  • Teams who need to collect the same complete information from everyone without writing code or paying for a custom build.
  • Anyone replacing chasing missing details over email with one structured record per enquiry.
  • Respondents are the people you send it to — the form asks them 17 questions across 1 screen.
  • Mobile-heavy audiences, thanks to the one-question-per-screen card layout.

Why this form is useful

It removes the cost of chasing missing details over email and turns each submission into a record your team can act on immediately.

  • 14 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It records a signature you can keep as proof of agreement.
  • It pins the request to a specific date.
  • Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
  • Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 3 conditional rules ship with it.

  • Features conditional validation: if “Tires condition” is Damaged or worn, “Describe the issue” becomes mandatory before the form can be sent.
  • Features conditional validation: if “Forks and mast condition” is Damaged or worn, “Describe the issue” becomes mandatory before the form can be sent.
  • Features conditional validation: if “Is the forklift safe to operate?” is No — took out of service, “Supervisor notified” becomes mandatory before the form can be sent.
  • All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

A forklift inspection is only useful if it happens before the truck moves, not after something goes wrong. This checklist follows the order most operators actually walk a truck: tires and forks first, since damage there is visible at a glance, then brakes, steering and the horn, then hydraulics, mast chains and fluid levels, and finally the warning devices — backup alarm, lights and seatbelt — that protect everyone else on the floor. Each item is a quick pass or flag rather than a paragraph, because a pre-shift check that takes fifteen minutes stops happening within a month. A flagged item automatically asks for a description, so a supervisor sees exactly what needs attention rather than a vague note. The closing question asks the operator directly whether the truck is safe to operate; a no takes the truck out of service until someone signs off on the fix, which matters more than any individual line item. Use one form per truck, per shift, and keep the completed checklists on file the way your safety program requires.

How the form changes as it's filled in

This template ships with 3 conditional rules, grouped into 3 behaviours driven by 3 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “Tires condition”

  • Makes answers required

If “Tires condition” is Damaged or worn, “Describe the issue” becomes required before the form can be submitted.

Driven by “Forks and mast condition”

  • Makes answers required

If “Forks and mast condition” is Damaged or worn, “Describe the issue” becomes required before the form can be submitted.

Driven by “Is the forklift safe to operate?”

  • Makes answers required

If “Is the forklift safe to operate?” is No — took out of service, “Supervisor notified” becomes required before the form can be submitted.

Set it up step by step

  1. 1

    Check before the engine starts

Tires, forks and visible damage are easiest to spot with the truck stationary and the key out.

  2. 2

    Make out-of-service a hard stop

A truck flagged unsafe should not go back into rotation until a supervisor signs off on the repair.

  3. 3

Keep one form per truck per shift

This makes it possible to see whether a fault was new or missed on the previous shift's check.

  4. 4

File checklists where you can find them

Inspection records are what auditors and insurers ask for first after an incident.

Mistakes to avoid

  • Filling out the checklist from memory at the end of a shift instead of walking the truck beforehand.
  • Letting a truck with a flagged fault stay in operation because no replacement is available.
  • Not naming a specific forklift ID, so faults can't be tracked to a particular unit over time.

What it pairs with

Most teams don't run this form on its own. These are the forms and systems it sits next to.

What to do with the responses

Take any truck marked unsafe out of service immediately, notify a supervisor, and log the repair before the truck returns to the floor.

Works in both layouts

These pages cover the same subject ground as the forklift inspection checklist, matched on the words people actually search for.

Privacy & compliance

This form collects health information. Keep access limited to the people who need it, turn on encrypted answers for the clinical questions, and set a retention window so records don't sit in the dashboard forever.

Frequently asked questions

How often should this be completed?

Most sites run it at the start of every shift a forklift is used, not just once a day if the truck runs across multiple shifts.

What happens if a fault is found mid-shift?

Stop using the truck, report it, and complete a fresh checklist once repairs are confirmed rather than editing the original record.

Can this replace formal maintenance records?

No — this is the operator-level daily check. Scheduled maintenance and repairs should be logged separately by whoever services the equipment.

Is this suitable for other powered equipment?

It's written specifically for forklifts; for general equipment loan tracking see the Equipment Checkout Form instead.
