Enrollment fails on administration far more often than on eligibility: a missing dependant date of birth, a start date nobody confirmed, or a member who did not say they were leaving another scheme. This three-page form closes those gaps. The first page covers the member: name, date of birth, address, contact details, employer and employment start date where the plan runs through work, and their identifying number in the scheme. The second page covers who else is being added, with each dependant's name, date of birth and relationship, and whether any of them lives at a different address. The third page covers the plan choice and the practical questions: which level of cover, the start date requested, any existing or previous health cover being replaced, a preferred doctor or clinic if the plan allows one, payment method and frequency, and the declarations including consent to share information with the insurer. The form does not describe what a plan covers or state whether a condition is excluded, because that depends on the scheme rules and the insurer's terms.
