Use this form to collect a reinstatement request for an existing policy that has lapsed or been canceled. It is an intake record, not confirmation that coverage has resumed. Put that distinction above the first question and repeat it in the submission receipt so a requested date is never mistaken for an approved effective date. Start with the policy number and policyholder contact details. Ask applicants to copy the lapse or cancellation date from their notice rather than estimate it; let them leave it blank if they cannot verify it. The reason for the lapse, payment status, and any relevant changes give the servicing team a useful starting point without turning the form into a full underwriting application. Keep payment evidence optional. A transaction reference or redacted receipt may help staff locate a payment, but this form should never collect card numbers, bank credentials, or detailed account statements.
