Who this template is for
Parking Permit Application is built for councils, agencies and public bodies who need to run a public request or application to a consistent standard.
- Councils, agencies and public bodies working in government & public sector.
- Teams who need to run a public request or application to a consistent standard without writing code or paying for a custom build.
- Anyone replacing paper forms that are slow to process and hard to audit with one structured record per enquiry.
- Respondents are residents and applicants — the form asks them 15 questions across 1 screen.
- Mobile-heavy audiences, thanks to the one-question-per-screen card layout.
Why this form is useful
It removes the cost of paper forms that are slow to process and hard to audit and turns each submission into a record your team can act on immediately.
- 10 questions are required, so submissions arrive complete instead of needing a follow-up email.
- It collects a verified email address so replies and confirmations land.
- It captures a phone number for urgent follow-up.
- It accepts a document or photo upload as evidence.
- It captures a full postal address.
- It captures a numeric value you can filter and sort on.
- Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
- Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint
Exactly how this form behaves as it is answered — 3 conditional rules ship with it.
- Features conditional validation: if “Are you the registered keeper of the vehicle?” is No, “Upload the lease or company letter” becomes mandatory before the form can be sent.
- Features advanced show logic: if “Which permit do you need?” is Visitor permit, the form dynamically exposes “How many visitor permits do you need?”. Otherwise that question never appears.
- Features conditional validation: if “Is this replacing a permit you already have?” is Yes, “Old permit number” becomes mandatory before the form can be sent.
- All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.
About this template
Parking permits are high-volume and repetitive, so the form should be one screen and reject nothing that a document can settle. This version asks who is applying, the address the permit is for and the parking zone if the resident knows it, then the vehicle: registration, make, model and colour, and whether the applicant is the registered keeper. That last question matters, because a lease or company car needs different proof and the applicant should find that out before they submit rather than a week later. It asks which type of permit is wanted, how long for, and whether this replaces an existing permit, with the old number if so. Two upload fields cover proof of address and proof that the vehicle belongs to the applicant. Zones, eligibility rules, permit counts per household and prices are set by each council, so check your own authority's rules and charges rather than assuming the ones nearby apply.

How the form changes as it's filled in
This template ships with 3 conditional rules, grouped into 3 behaviours driven by 3 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.
Driven by “Are you the registered keeper of the vehicle?”
- Makes answers required
If “Are you the registered keeper of the vehicle?” is No, “Upload the lease or company letter” becomes required before the form can be submitted.
Driven by “Which permit do you need?”
- Reveals questions
If “Which permit do you need?” is Visitor permit, the form reveals “How many visitor permits do you need?”.
Driven by “Is this replacing a permit you already have?”
- Makes answers required
If “Is this replacing a permit you already have?” is Yes, “Old permit number” becomes required before the form can be submitted.
Set it up step by step
- 1
Ask for the zone but do not require it
Most residents do not know their zone, and you can look it up from the address faster than they can find it.
- 2
Handle non-keepers with one upload
Lease and company cars are a large minority of applications, and the conditional upload settles them without an email exchange.
- 3
Publish eligibility rules beside the form
Household permit limits are the main reason applications are refused, so state yours before people apply.
- 4
Confirm with the permit reference
Send the reference and the start date in the acknowledgement; most follow-up contacts are people asking for exactly that.
Mistakes to avoid
- Requiring the parking zone, which turns a two-minute form into a search of the council website.
- Not asking about the registered keeper, so every lease car becomes a manual follow-up.
- Printing prices in the form when charges are set annually.
What it pairs with
Most teams don't run this form on its own. These are the forms and systems it sits next to.
What to do with the responses
Check the proof of address against the zone, issue the permit with a reference and start date, and cancel any permit named in the replacement field.

Frequently asked questions
How is this different from the general permit form?
The Permit Application Form is a flexible template for many permit types. This one is shaped around parking: the zone, the vehicle, the keeper and proof of address.
Can someone apply for a lease or company car?
Yes. Answering no to the keeper question asks for the lease or company letter, which is what most councils need for those vehicles.
What proof of address is accepted?
That is set locally, so list what you accept beside the upload field rather than in the form's help text where it will go out of date.
Does it handle renewals?
Yes. The replacement question and the old permit number let one form cover both a new application and a renewal without a second workflow.
Is the Parking Permit Application template free to use?
Yes. You can preview and test Parking Permit Application on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.
What does the Parking Permit Application template ask for?
It asks 15 questions across 1 page, 10 of which are required. Every question is listed in full further down this page, and each one can be renamed, reordered, made optional or deleted after you copy the template.
Can I edit the Parking Permit Application form after copying it?
Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the classic and card layouts, restyle it to match your brand, and set who gets notified on each submission.

3 conditional rules ship with the template: Features conditional validation: if “Are you the registered keeper of the vehicle?” is No, “Upload the lease or company letter” becomes mandatory before the form can be sent. Features advanced show logic: if “Which permit do you need?” is Visitor permit, the form dynamically exposes “How many visitor permits do you need?”. Otherwise that question never appears. Features conditional validation: if “Is this replacing a permit you already have?” is Yes, “Old permit number” becomes mandatory before the form can be sent. All rules are editable in the Logic tab.
Where do responses to the Parking Permit Application form go?
Submissions land in your workspace under Submissions, where you can search, filter, label and export them to CSV or PDF. You can also email a notification to your team on every submission and send the respondent an auto-reply.
Can I embed the Parking Permit Application form on my own website?
Yes. Publish the form and paste the embed snippet into any page or share the direct link. The embedded form resizes to fit its container and loads no advertising or marketing trackers inside the iframe.
Can respondents upload files?
Yes. This template includes a file upload question, so respondents can attach documents or photos with their answers. You can adjust the accepted file types and size limits on that question.
