About this template
Referrals get returned for the same reasons every time: no clear question, missing history, or no way to reach the referrer. This form is built to prevent all three. It captures the referring clinician and their direct contact route first, then the patient's identifiers and how the service should contact them, including any interpreter or access need. The clinical section asks for the reason for referral in a sentence, the relevant history, current medication, what has already been tried and what happened, and any investigations already done with room to attach results. The question that changes the quality of a referral most is the explicit one about what the referrer is asking for: an opinion, joint management, a procedure, or a specific test. Urgency is asked with a supporting note rather than as a bare category. Because the service can define the fields it needs, a receiving team stops rewriting incomplete referrals and starts triaging them, and the referrer gets an acknowledgement with a reference instead of silence.
