About this template

It takes eight questions: name, email, phone, how many tickets, which draw, how they are paying, where they heard about the raffle and a confirmation that they meet the age and eligibility rules for your jurisdiction. That last question is not decoration — most places that permit raffles set an age limit and residency conditions, and having a timestamped confirmation for every entry is what makes the draw defensible. It complements the Raffle Ticket Order Form, which is built around bulk book sales and seller reconciliation; this one is for individual online entries. Two rules handle the edge cases: answering no to eligibility shows a note explaining the entry cannot be accepted, and choosing online payment reveals the payment-link note so the entrant knows what happens next.
