Who this template is for
Alumni Mentoring Sign-up Form is built for small teams who need a reliable intake process who need to collect the same complete information from everyone.
- Small teams who need a reliable intake process working in fundraising & alumni.
- Teams who need to collect the same complete information from everyone without writing code or paying for a custom build.
- Anyone replacing chasing missing details over email with one structured record per enquiry.
- Respondents are the people you send it to — the form asks them 19 questions across 4 screens.
Why this form is useful
It removes the cost of chasing missing details over email and turns each submission into a record your team can act on immediately.
- 14 questions are required, so submissions arrive complete instead of needing a follow-up email.
- It collects a verified email address so replies and confirmations land.
- It allows multiple selections without free text.
- It captures a numeric value you can filter and sort on.
- Conditional logic hides 4 questions until they are relevant — shorter forms convert better than long ones.
- Splitting the form across 4 pages keeps each screen short and shows respondents how much is left.
- Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint
Exactly how this form behaves as it is answered — 4 conditional rules ship with it.
- Features advanced show logic: if “How would you prefer to meet?” is In person, the form dynamically exposes “How far are you willing to travel?”. Otherwise that question never appears.
- Features advanced show logic: if “Are you willing to mentor students under 18?” is Yes, the form dynamically exposes “Do you hold a current background check?”. Otherwise that question never appears.
- Features advanced show logic: if “Do you hold a current background check?” is No, the form dynamically exposes “We will need to arrange a check before any introduction”. Otherwise that question never appears.
- Features conditional validation: if “Can you offer an introduction to your network?” is Yes, “What kind of introductions could you make?” becomes mandatory before the form can be sent.
- All 4 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.
About this template
Mentoring programmes fail on capacity far more often than on goodwill. This form asks the awkward questions up front so the match holds. A mentor states their sector, their route into it and the kind of support they can offer — CV review, mock interviews, ongoing career conversations, portfolio feedback or an introduction to their own network — and then how many hours a month and how many mentees they can genuinely take. Meeting preference is captured because an in-person-only mentor two hundred miles from campus is not a match, and choosing in person opens the travel radius. If the programme involves students under eighteen the mentor is asked about existing background checks, and answering no opens a short explanation of what will be required before an introduction can be made. Language and accessibility needs are asked once, plainly. The last page collects the short profile paragraph students actually read when choosing a mentor, which is the field most programmes forget to gather and then have to write themselves from a CV.

Set it up step by step
- 1
Match your sectors
Set the sector list to the fields your students actually ask about.
- 2
Be honest about capacity
Keep the hours and mentee-count questions required — they prevent over-matching.
- 3
Handle safeguarding
If any mentee could be under 18, keep the background check branch and complete it before introductions.
- 4
Collect the profile
Use the mentor's own paragraph in the matching list rather than writing one from their CV.
- 5
Close the loop
Confirm each pairing to both sides in one email with a suggested first meeting date.
Mistakes to avoid
- Recruiting mentors without asking how many hours a month they have, then matching them to three students each.
- Matching only on job title, when what students want is the sector route and the first-job story.
- Skipping the safeguarding question for programmes that involve under-18s.

Most teams don't run this form on its own. These are the forms and systems it sits next to.
