This form replaces it with the six answers that actually set a price: what the piece is, the style, the size, how many characters or subjects appear, whether there is a background, and what the client intends to do with the finished art. References are required rather than optional, because a described style and a shown style are rarely the same thing. The second page handles the commercial side — budget range, deadline and usage — so a personal keepsake and a product illustration never get quoted the same way. Two rules do the work an artist would otherwise do by hand: choosing 'Other' as a style opens a required description, and choosing commercial usage opens a required licensing question along with a note that commercial rates differ.
