About this template

A model release is a commercial agreement as much as a consent, and this form captures both sides of it. The model provides their name, professional name, agency if represented, contact details and date of birth, with guardian consent required for minors. The shoot is documented with its title, date, location, photographer and client, and a description of the concept including any nudity, partial nudity or sensitive content, which is disclosed explicitly rather than assumed. Usage rights are itemised: editorial, commercial advertising, packaging, social media, stock licensing and AI training are separate choices, so nothing is granted by silence. Duration, territory and exclusivity are set, along with whether the images may be edited, retouched or composited. Payment terms record the agreed fee or the fact that the shoot is on a collaborative basis, the invoice arrangement and any usage buyout. Credit requirements are captured so the model's name or handle is used as agreed. Both parties' details, the signature and the timestamp form the executed record.
