About this template

Body art studios need a defensible consent record for every session, and this waiver captures identity, medical fitness, design agreement and aftercare acknowledgement in one signed document. The client confirms their full name, date of birth and that photo ID has been shown, then works through a medical checklist covering blood-borne conditions, diabetes, epilepsy, heart conditions, skin disorders, pregnancy or breastfeeding, blood thinners, allergies to latex, inks or antiseptics, and any recent alcohol or drug use. Positive answers open a details box rather than silently passing. The design section records placement, size, whether it is a cover-up or reworking of existing work, the artist assigned and the agreed price or deposit. Aftercare is not a leaflet handed over at the door — the client explicitly confirms it has been explained and that they accept the risk of infection if it is ignored. The declaration then covers photography permission for your portfolio, refusal of service if intoxicated, and a signature captured on screen with a timestamp.
