This form is the reply, written so it can be completed at the chair. It records the patient, their date of birth, the referring physician and the procedure, why clearance is needed — surgery, oncology, cardiac, orthopaedic or other — and the date of the dental examination itself. Then it records findings, whether any infections or extractions are needed, and the clearance decision: cleared, cleared after treatment, or not cleared. The dentist signs at the bottom. It is a narrower document than the Dental New Patient Intake Form, which builds a full history for ongoing care; this exists to answer one question for one procedure, and it should be readable in under a minute by whoever receives it.
