Who this template is for

Health & Safety Induction Quiz Form is built for small teams who need a reliable intake process who need to collect the same complete information from everyone.

  • Small teams who need a reliable intake process working in quizzes & assessments.
  • Teams who need to collect the same complete information from everyone without writing code or paying for a custom build.
  • Anyone replacing chasing missing details over email with one structured record per enquiry.
  • Respondents are the people you send it to — the form asks them 19 questions across 3 screens.

Why this form is useful

It removes the cost of chasing missing details over email and turns each submission into a record your team can act on immediately.

  • 15 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It captures a phone number for urgent follow-up.
  • It accepts a document or photo upload as evidence.
  • It records a signature you can keep as proof of agreement.
  • It pins the request to a specific date.
  • It allows multiple selections without free text.
  • Conditional logic hides 5 questions until they are relevant — shorter forms convert better than long ones.
  • Splitting the form across 3 pages keeps each screen short and shows respondents how much is left.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 5 conditional rules ship with it.

  • Features advanced show logic: if “Which best describes you?” is Contractor, the form dynamically exposes “Attach your method statement”. Otherwise that question never appears.
  • Features advanced show logic: if “Which best describes you?” is Contractor, the form dynamically exposes “Who is escorting you on site?”. Otherwise that question never appears.
  • Features advanced show logic: if “Do you have a condition that affects how you evacuate?” is Yes, the form dynamically exposes “Details for your personal evacuation plan”. Otherwise that question never appears.
  • Features advanced show logic: if “Induction outcome” is Re-induct, the form dynamically exposes “Re-induction date”. Otherwise that question never appears.
  • Features conditional validation: if “Induction outcome” is Re-induct, “Re-induction date” becomes mandatory before the form can be sent.
  • All 5 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

An induction that is not tested is a signature on a sheet, and a signature does not evacuate a building. This quiz asks the questions that matter on the day someone first walks onto your site: where their nearest fire exit and assembly point are, who the first aiders and fire marshals are, what the alarm sounds like, what they do if they find a hazard, and which PPE is mandatory in the areas they will actually enter. It captures whether the person is an employee, an agency worker or a contractor, because a contractor's answers open extra questions about their own method statement and the escort arrangements they will work under. Anyone declaring a medical condition or accessibility need that affects evacuation is asked for the detail so a personal emergency evacuation plan can be raised rather than assumed. The inductor is named and signs, the outcome is recorded as pass or re-induct, and a re-induct answer forces the date it will happen — which is the difference between a safety record and a filing exercise.

How the form changes as it's filled in

This template ships with 5 conditional rules, grouped into 4 behaviours driven by 3 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “Which best describes you?

  • Reveals questions

If “Which best describes you?” is Contractor, the form reveals “Attach your method statement” and “Who is escorting you on site?”.

Driven by “Do you have a condition that affects how you evacuate?

  • Reveals questions

If “Do you have a condition that affects how you evacuate?” is Yes, the form reveals “Details for your personal evacuation plan”.

Driven by “Induction outcome

  • Reveals questions

If “Induction outcome” is Re-induct, the form reveals “Re-induction date”.

  • Makes answers required

If “Induction outcome” is Re-induct, “Re-induction date” becomes required before the form can be submitted.

Set it up step by step

  1. 1

    Copy the template

Rename it for the site, and put the building's name in the form title so records cannot be confused between locations.

  2. 2

    Localise every answer

Replace the exits, assembly points, marshals and first aiders with the real ones for that building.

  3. 3

    Induct in person, record here

Walk the route first, then complete the quiz — the form evidences understanding, it does not replace the walk.

  4. 4

    Gate the badge

Only issue access once the outcome is Pass, and keep the record with the access log.

  5. 5

    Re-run annually

Re-induct on a yearly cycle and whenever the layout, marshals or hazards change.

Mistakes to avoid

  • Inducting on generic content instead of the hazards of the actual building.
  • Passing someone who could not name their fire assembly point.
  • Not recording the inductor, so nobody owns the sign-off.

What it pairs with

Most teams don't run this form on its own. These are the forms and systems it sits next to.

What to do with the responses

Issue the site badge or access card only against a passed record, and re-induct anybody whose answers about escape routes or first aid were wrong.

Works in this layout

These pages cover the same subject ground as the health & safety induction quiz form, matched on the words people actually search for.

Privacy & compliance

This form collects health information. Keep access limited to the people who need it, turn on encrypted answers for the clinical questions, and set a retention window so records don't sit in the dashboard forever.

Frequently asked questions

Can a contractor complete this before arriving?

They can complete the personal details and method statement in advance, but the site-specific answers should be given after the physical walk-round.

What outcome should a partly wrong quiz get?

Re-induct. If somebody cannot name their assembly point, the correct action is another walk, not a note in the margin.

Do we need a separate permit as well?

Yes, for any controlled work. Induction confirms they know the site; a permit authorises a specific job — pair this with the Permit to Work Form.

How do we handle evacuation needs?

Any declared condition opens a detail box so a personal emergency evacuation plan can be written with the person, not about them.
