Compliance checklist

Ready to publish
  • Payment items priced — passingEvery payment question charges a real amount, so a submission always records a payment.
  • Conditional logic resolves — passingEvery rule points at a question or page that exists in this template.
  • Legal text not required — passingThis template collects neither health nor regulated financial information.
  • Question labels unique — passingLogic and exports address questions by label, so duplicates are ambiguous.

What happens after submit: nothing is stored in this preview. On your own copy, every response triggers an instant email notification and lands in your response dashboard, ready to export or forward.

Who this template is for

Multiple Choice Test is built for small teams who need a reliable intake process who need to collect the same complete information from everyone.

  • Small teams who need a reliable intake process working in quizzes & assessments.
  • Teams who need to collect the same complete information from everyone without writing code or paying for a custom build.
  • Anyone replacing chasing missing details over email with one structured record per enquiry.
  • Respondents are the people you send it to — the form asks them 15 questions across 2 screens.
  • Mobile-heavy audiences, thanks to the one-question-per-screen card layout.

Why this form is useful

It removes the cost of chasing missing details over email and turns each submission into a record your team can act on immediately.

  • 12 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It scores satisfaction on a consistent scale.
  • Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
  • Splitting the form across 2 pages keeps each screen short and shows respondents how much is left.
  • Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 3 conditional rules ship with it.

  • Features advanced show logic: if “Question 10” is answered, the form dynamically exposes “How confident are you?”. Otherwise that question never appears.
  • Features conditional validation: if “Question 10” is answered, “How confident are you?” becomes mandatory before the form can be sent.
  • Features advanced show logic: if “Class or group” is Other, the form dynamically exposes “Which group are you in?”. Otherwise that question never appears.
  • All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

Most people searching for a multiple choice test want the shape of the paper, not a blank builder: ten questions, four options each, an identity page at the front and consistent wording throughout. That is exactly what this template ships. Page one captures who is sitting the test — name, email or student ID, and class or group — so results can be matched to a register. Page two carries ten single-answer questions written on general knowledge, deliberately easy to overwrite with your own subject matter while keeping the option count and phrasing consistent. It differs from the Fill-in-the-Blanks Quiz in what it measures: this one tests recognition, where a learner picks the right answer from a set, while fill-in-the-blanks tests recall, where nothing is offered and the answer has to be produced. Recognition tests are quicker to mark and better for coverage; recall tests are harder to guess. A confidence rating appears once the final question is answered, which tells you whether a right answer was knowledge or luck.

How the form changes as it's filled in

This template ships with 3 conditional rules, grouped into 3 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “Question 10

  • Reveals questions

If “Question 10” is answered, the form reveals “How confident are you?”.

  • Makes answers required

If “Question 10” is answered, “How confident are you?” becomes required before the form can be submitted.

Driven by “Class or group

  • Reveals questions

If “Class or group” is Other, the form reveals “Which group are you in?”.

Set it up step by step

  1. 1

    Swap in your subject

Replace the ten placeholder questions with your own, keeping four options each so the paper stays consistent.

  2. 2

    Vary the answer position

Move the correct option around the four slots so position alone never gives the answer away.

  3. 3

    Turn on scoring

In the builder, mark the correct option on each question and set a pass mark to grade submissions automatically.

  4. 4

    Decide on identity

Keep the student ID field if results feed a register; delete it for an anonymous practice paper.

  5. 5

    Set a time expectation

Say in the cover text roughly how long the paper takes so nobody abandons it halfway.

Mistakes to avoid

  • Writing four options where two are obviously silly — a test with two plausible answers is a coin flip, not an assessment.
  • Letting the correct answer sit in the same position repeatedly, which lets a guesser out-score a learner.
  • Collecting names but no class or student ID, so two learners called James cannot be told apart at marking time.

What it pairs with

Most teams don't run this form on its own. These are the forms and systems it sits next to.

What to do with the responses

Export the responses to CSV and mark them in one pass, or turn the radio questions into scored options in the builder so every submission arrives with a total and a pass or fail already attached.

Works in both layouts

These pages cover the same subject ground as the multiple choice test, matched on the words people actually search for.

Frequently asked questions

Does it mark itself?

Not as shipped. Mark the correct option on each question in the builder and add a pass mark, and every submission arrives scored.

Can I add more than ten questions?

Yes. Duplicate any question card and edit it — the layout and logic carry over, so a twenty-question paper takes a couple of minutes.

How is this different from the fill-in-the-blanks quiz?

This tests recognition, where the answer is on screen. Fill-in-the-blanks tests recall, where it isn't. Use both across a course for a fuller picture.

Can I stop people going back and changing answers?

Use the card layout with one question per screen and turn off back navigation in the builder settings.

Is the Multiple Choice Test template free to use?

Yes. You can preview and test Multiple Choice Test on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.

What does the Multiple Choice Test template ask for?

It asks 15 questions across 2 pages, 12 of which are required. Every question is listed in full further down this page, and each one can be renamed, reordered, made optional or deleted after you copy the template.

Can I edit the Multiple Choice Test form after copying it?

Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the card and classic layouts, restyle it to match your brand, and set who gets notified on each submission.

How does the conditional logic in this quizzes & assessments form work?

3 conditional rules ship with the template: Features advanced show logic: if “Question 10” is answered, the form dynamically exposes “How confident are you?”. Otherwise that question never appears. Features conditional validation: if “Question 10” is answered, “How confident are you?” becomes mandatory before the form can be sent. Features advanced show logic: if “Class or group” is Other, the form dynamically exposes “Which group are you in?”. Otherwise that question never appears. All rules are editable in the Logic tab.

Where do responses to the Multiple Choice Test form go?

Submissions land in your workspace under Submissions, where you can search, filter, label and export them to CSV or PDF. You can also email a notification to your team on every submission and send the respondent an auto-reply.

Can I embed the Multiple Choice Test form on my own website?

Yes. Publish the form and paste the embed snippet into any page or share the direct link. The embedded form resizes to fit its container and loads no advertising or marketing trackers inside the iframe.
