Consent for a vaccination has to be taken before the dose and has to show that screening questions were asked. This form does both in a sequence that works at the front of a busy clinic. The first page names the vaccine being offered, confirms who the dose is for, and asks the screening questions: previous reaction to this vaccine or its components, known allergies, current illness or fever, immune-suppressing treatment, pregnancy where applicable, and any bleeding disorder. Each answer that needs detail opens a short box rather than being left as a bare yes. The second page records that written information about the vaccine was provided and that questions were invited and answered, then takes the signature. Where the person receiving the dose is a child or an adult who lacks capacity to consent, the form captures the signer's relationship and authority instead of assuming a parent is present. Once signed, the dose itself is recorded separately, which keeps the consent document clean and makes both halves easy to produce at audit.
