About this template
Triage is a decision, and a triage form should record how the decision was reached. This one is completed by the clinician, not the patient. It opens with the patient's identifiers, the time of contact and the route the contact came in by, then the presenting problem in the patient's words and its duration. The observations section holds whatever your service records at triage, with space for the values rather than a judgement about them. A red-flag section is a deliberate checklist for the presentation, because the value of triage documentation is showing which specific concerns were considered and excluded. The clinician then records the advice given, whether safety-netting was explained and what the patient was told to watch for, and finally the disposition: self-care, routine appointment, same-day appointment, home visit or onward escalation. Choosing escalation requires a note about where and when. Because every submission is timestamped and searchable, the list doubles as an audit trail for how a busy morning was managed rather than a set of loose call notes.
