Who this template is for

Marketing Opt-In Form is built for any team that needs demonstrable permission who need to record who agreed to what, and when.

  • Any team that needs demonstrable permission working in consent, waivers & releases.
  • Teams who need to record who agreed to what, and when without writing code or paying for a custom build.
  • Anyone replacing consent claims you can't evidence months later with one structured record per enquiry.
  • Respondents are customers, patients and participants — the form asks them 10 questions across 1 screen.
  • Mobile-heavy audiences, thanks to the one-question-per-screen card layout.

Why this form is useful

It removes the cost of consent claims you can't evidence months later and turns each submission into a record your team can act on immediately.

  • 3 questions are required, so submissions arrive complete instead of needing a follow-up email.
  • It collects a verified email address so replies and confirmations land.
  • It captures a phone number for urgent follow-up.
  • It pins the request to a specific date.
  • It captures a full postal address.
  • It allows multiple selections without free text.
  • Conditional logic hides 2 questions until they are relevant — shorter forms convert better than long ones.
  • Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

Logic and conditions blueprint

Exactly how this form behaves as it is answered — 2 conditional rules ship with it.

  • Features advanced show logic: if “Which channels would you like to hear from us on?” is one of Email, SMS / text message, the form dynamically exposes “Topics you're interested in”. Otherwise that question never appears.
  • Features conditional validation: if “Where did this request come from?” is Other, “Please specify” becomes mandatory before the form can be sent.
  • All 2 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.

About this template

PECR treats electronic marketing consent separately from the general lawful basis a business relies on for other processing, and a single "yes, market to me" checkbox does not satisfy either. This form asks for consent channel by channel — email, SMS and post are each their own unticked box, because a person agreeing to a newsletter has not automatically agreed to text messages. It records who is consenting, when, and which organisation they are consenting to hear from, plus an optional note on where the consent was collected (in branch, online, at an event) so a later audit can trace the source. A topics-of-interest field lets people narrow what they hear about without withdrawing consent entirely. The closing section states plainly that consent can be withdrawn at any time and how, which is expected practice rather than an optional extra. None of the boxes are pre-ticked, none are bundled with an unrelated action like completing a purchase, and the record this form produces — timestamp, channel, wording shown — is the kind of evidence a regulator or an internal audit will actually ask to see.

How the form changes as it's filled in

This template ships with 2 conditional rules, grouped into 2 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.

Driven by “Which channels would you like to hear from us on?

  • Reveals questions

If “Which channels would you like to hear from us on?” is one of Email, SMS / text message, the form reveals “Topics you're interested in”.

Driven by “Where did this request come from?

  • Makes answers required

If “Where did this request come from?” is Other, “Please specify” becomes required before the form can be submitted.

Set it up step by step

  1. 1

    Never pre-tick a channel

Each checkbox must start empty — PECR consent has to be a positive, freely given action, not a default someone has to notice and undo.

  2. 2

    Keep channels separate

Email, SMS and post have different rules and different withdrawal routes, so record and manage them as separate consents, not one combined flag.

  3. 3

    Log the source

The "where did this request come from" field matters more than it looks — it is what lets you show a consent record actually came from the person, not an imported list.

  4. 4

Make withdrawal as easy as opt-in

Every marketing email should carry an unsubscribe link that maps back to the channel recorded here, not just a general "contact us" address.

Mistakes to avoid

  • Using one combined "marketing" checkbox for email, SMS and post, which cannot show separate consent if a regulator asks about a specific channel.
  • Pre-ticking any of the channel boxes, or bundling consent into a purchase or event registration so it isn't a distinct choice.
  • Not recording the date and source of consent, leaving no evidence trail if the consent is ever challenged.

What it pairs with

Most teams don't run this form on its own. These are the forms and systems it sits next to.

What to do with the responses

Store each channel's consent as a separate, timestamped record against the contact, and suppress any channel the person didn't tick from your marketing sends immediately.

Works in both layouts

These pages cover the same subject ground as the marketing opt-in form, matched on the words people actually search for.

Privacy & compliance

This form collects identity or financial details. Enable encrypted answers on the sensitive fields, avoid emailing full values in notifications, and delete responses once the check is complete.

Frequently asked questions

Is this the same as a general GDPR consent form?

No — the GDPR Consent & Data Request Form covers broader data processing consent, while this one is specifically about PECR's electronic marketing rules, which need their own, separate, unticked opt-in per channel.

Do I need a lawful basis form as well?

Yes if you also process the same data for a non-marketing purpose — pair this with the Lawful Basis Record so the marketing consent and the underlying processing basis are documented separately.

Can someone opt in to SMS only?

Yes, that's the point of separate checkboxes — they can tick SMS and leave email and post blank, and only SMS marketing should follow.

How long should I keep this record?

Keep it for as long as you rely on the consent, and be ready to produce the timestamp and wording shown if someone later disputes having agreed.

Is the Marketing Opt-In Form template free to use?

Yes. You can preview and test Marketing Opt-In Form on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.

Can I edit the Marketing Opt-In Form form after copying it?

Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the classic and card layouts, restyle it to match your brand, and set who gets notified on each submission.

How does the conditional logic in this consent, waivers & releases form work?

2 conditional rules ship with the template: Features advanced show logic: if “Which channels would you like to hear from us on?” is one of Email, SMS / text message, the form dynamically exposes “Topics you're interested in”.
