Scope is the part most templates get wrong, so it is explicit here: the subject chooses which uses they permit — website, social media, print marketing, press, advertising, internal use or third-party licensing — and whether their name may be published alongside the image. Duration options cover a single campaign, a fixed number of years, or perpetual use, and the territory can be limited. The subject confirms whether payment or consideration has been given and whether they retain any right to approve images. The declaration confirms they have read the release and understand it, and is signed and dated on screen.
