About this template
A waiver only helps if it names the activity, the risks and the participant clearly, and if you can prove when it was signed. This template is a general-purpose waiver you can adapt to any activity from a climbing session to a charity run. The participant gives their full name, date of birth, address and contact details, plus an emergency contact and any relevant medical conditions, allergies or medication. The activity is described specifically — name, date, location and operator — and the risks are listed rather than implied, with a space to add activity-specific hazards such as water, height, contact or equipment failure. The participant then confirms they are physically fit to take part, that they have disclosed anything relevant, that they will follow safety instructions and use provided equipment properly, and that they accept responsibility for their own belongings. Minors require a guardian signature. Because the signature, timestamp and full text are stored together, you have a complete, dated waiver record for every participant.
