About this template
Membership applications go wrong in the gap between joining and being approved. This form closes it. The applicant gives their contact details and chooses a membership tier, and the tier drives the rest: a student or concession tier asks for proof of eligibility, a family tier opens fields for the additional members to be covered, and a corporate tier asks for the organisation and the number of named seats. Professional associations get an eligibility block for qualifications, registration numbers and years in the field, which appears only where the tier requires vetting. Proposer and seconder fields cover clubs that still need two existing members to support an application, and can be deleted in one action for those that do not. Dues are stated rather than implied: the annual amount, whether the applicant wants to pay yearly or monthly, and the start date they expect membership to run from. The declaration page covers the constitution or code of conduct, a photography and communications preference, and a signature. What arrives is an application a committee can approve without a follow-up email.
