A gym joining form has to do three jobs at once: sell the right membership, screen for health risk and record a waiver that stands up. This template handles all three in one pass. The member enters contact details, date of birth and an emergency contact, then chooses a plan — off-peak, full access, class pass, annual paid up front or a student or corporate rate — with add-ons for personal training blocks, locker hire and guest passes. A pre-exercise health questionnaire follows the standard screening pattern: chest pain, heart conditions, blood pressure, dizziness, joint problems, pregnancy, medication and any advice from a doctor to avoid exercise. Any positive answer requires detail and flags the member for a GP clearance conversation before their induction. Goals and experience questions let the induction coach prepare a sensible starting programme rather than a generic tour. Finally the member accepts the assumption-of-risk waiver, the minimum term and notice period, and signs on screen, giving you a dated membership record.
