About this template

Whether you are an agency building a roster or a family hiring directly, the questions that matter about a babysitter are consistent, and this application form asks them in a defensible order. The applicant gives contact details, date of birth, right-to-work status and whether they drive, with licence and insurance details where transporting children is involved. Experience is captured by age band — babies, toddlers, primary and teenagers — with the number of years, the settings they have worked in and the largest number of children they have looked after at once. Qualifications and training cover childcare qualifications, paediatric first aid with expiry date, safeguarding training and food hygiene. Screening questions ask about background checks with reference numbers, any prohibition from working with children, and consent to further checks. Availability is recorded by day and time including overnight and short notice. Two professional references with dates of employment are required, and the applicant signs a declaration confirming the accuracy of everything given.
