This form is for baptisms only, and it collects what the register and the preparation both need. Where the candidate is a child, it is completed by a parent or guardian. The first page covers the candidate: full name and how it should be recorded, date and place of birth, whether this is an infant, child or adult baptism, and whether they have been baptised before. It records the parents or, for an adult, the candidate's own details, along with the family's connection to the congregation and whether they are members or attend elsewhere. The second page covers the people and the day: godparents or sponsors with their names and whether they have been baptised themselves, preparation class attendance and which sessions, the preferred date with alternatives, whether the baptism should be within a regular service or separately, expected number of guests, whether photography is wanted, accessibility needs for anyone attending, and any family circumstances the minister should know before the day. Wording is kept denomination-neutral except where a field genuinely needs otherwise.
