About this template
Ceremony enquiries arrive by phone, email and after the service, and this request form gives clergy and administrators a single consistent record for every one. The enquirer chooses the ceremony type — wedding, blessing, baptism or christening, confirmation, dedication, or a funeral or memorial — and the form then asks only the questions that ceremony needs. Weddings capture both partners' names, contact details, preferred dates with alternatives, whether either has been married before, the expected number of guests and whether banns or a licence are needed. Baptisms ask for the child's full name and date of birth, the parents' details, proposed godparents and any family connection to the church. Every enquiry records the enquirer's relationship to the congregation, whether they live in the parish, and which minister they have already spoken to. A preparation section explains that meetings are required beforehand and asks for availability. The result is an enquiry the office can answer with a date rather than a list of further questions.
