About this template
Online giving works best when the form is short, respectful and obvious about where the money goes. This card-style form asks the giver to choose an amount from suggested tiers or enter their own, then designate the gift to a fund — general offering, tithe, building fund, missions, benevolence or a named appeal. A frequency question distinguishes one-off gifts from weekly or monthly regular giving, and monthly givers see a start date so the first collection is never a surprise. Contact details are collected so gifts can be acknowledged and, where relevant, recorded against a giving number for year-end statements. A gift aid or tax relief declaration appears as its own step with the exact wording your jurisdiction requires, including the confirmation that the giver pays enough tax to cover the claim. Optional questions let people dedicate a gift in memory of someone, keep the gift anonymous from published thank-yous, or add a note to the pastoral team. Receipts are sent automatically on submission.
