About this template
Most congregations keep their membership records in a spreadsheet that nobody trusts, and this registration form is the cleanest way to rebuild that list from the people themselves. It collects the member's name, contact details, address and date of birth, then household information: partner or spouse, children with ages, and who should be treated as the main contact for the household. A church life section records how long they have attended, whether they have been baptised or confirmed and where, any previous congregation, and whether they wish to be received into formal membership now or later. Ministry interests are captured as checkboxes covering music, welcome team, children and youth, hospitality, prayer, technical and outreach, which immediately gives ministry leaders a list to follow up. A pastoral notes box lets people share anything they would like the pastoral team to know. Data consent is explicit and separated by purpose — directory listing, newsletter, rota contact and photography — so your records stay defensible.
