About this template
An address change looks trivial until it is done badly: post follows the old address, invoices bounce, and payroll files a P60 to a house someone left in March. This single-page form removes the guesswork by asking for four things at once — who is moving, which record it applies to, both addresses in full, and the date the change takes effect. The reference field matters more than it looks; without it, an administrator has to search on a name that may appear three times. A notify list closes the loop by naming the teams that must be told, so nobody assumes billing already knows. It differs from the Address Collection Form, which gathers an address for the first time from a new contact — this one amends an address that is already on file, and it records the before as well as the after. Two rules keep it short: a changed phone number opens a required new number, and choosing billing surfaces a note about invoice addresses needing separate approval.
