Compliance checklist
- Payment items priced — passingEvery payment question charges a real amount, so a submission always records a payment.
- Conditional logic resolves — passingEvery rule points at a question or page that exists in this template.
- Legal text not required — passingThis template collects neither health nor regulated financial information.
- Question labels unique — passingLogic and exports address questions by label, so duplicates are ambiguous.
What happens after submit: nothing is stored in this preview. On your own copy, every response triggers an instant email notification and lands in your response dashboard, ready to export or forward.

Who this template is for
Address Collection Form is built for sales teams, agencies and independent consultants who need to qualify inbound demand before anyone spends time on a call.
- Sales teams, agencies and independent consultants working in lead generation & sales.
- Teams who need to qualify inbound demand before anyone spends time on a call without writing code or paying for a custom build.
- Anyone replacing unqualified enquiries clogging the pipeline with one structured record per enquiry.
- Respondents are prospects arriving from ads, search and referrals — the form asks them 10 questions across 1 screen.
- Mobile-heavy audiences, thanks to the one-question-per-screen card layout.
Why this form is useful
It removes the cost of unqualified enquiries clogging the pipeline and turns each submission into a record your team can act on immediately.
- 3 questions are required, so submissions arrive complete instead of needing a follow-up email.
- It collects a verified email address so replies and confirmations land.
- It captures a phone number for urgent follow-up.
- It captures a full postal address.
- Conditional logic hides 2 questions until they are relevant — shorter forms convert better than long ones.
- Every response is stored, searchable and exportable, so nothing depends on one person's inbox.

- 3Open Notifications and add the email addresses that should be alerted on each submission; add an auto-reply to the respondent if the form collects an email address.

Logic and conditions blueprint
Exactly how this form behaves as it is answered — 2 conditional rules ship with it.
- Features advanced show logic: if “This address is for” is Other, the form dynamically exposes “Tell us what for”. Otherwise that question never appears.
- Features advanced show logic: if “OK to share with the event organiser?” is No, the form dynamically exposes “How we use your address”. Otherwise that question never appears.
- All 2 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.
About this template
The classic version of this job is a group chat where twelve people paste half an address each. This form replaces it. A structured address block keeps house number, street, town and postcode in separate fields, so the export drops straight into a label template without anyone retyping it. Name, email and phone give you a way to chase the two people who never reply, and an address type field tells you whether a parcel will be signed for at work or left on a doorstep. Weddings and holiday cards are the leading use, which is why the purpose dropdown leads with them, but the same form handles shipping lists and newsletter sign-ups. Two rules keep it polite: choosing 'Other' opens a short explanation, and declining to share with an event organiser surfaces a privacy note so the person knows what happens next. Collect once, use it for the save-the-dates and the thank-you cards, then delete it.

How the form changes as it's filled in
This template ships with 2 conditional rules, grouped into 2 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.
Driven by “This address is for”
- Reveals questions
If “This address is for” is Other, the form reveals “Tell us what for”.
Driven by “OK to share with the event organiser?”
- Reveals questions
If “OK to share with the event organiser?” is No, the form reveals “How we use your address”.
Set it up step by step
- 1
Say what it is for
Put one line in the intro explaining the mailing — people complete address forms far more readily when the purpose is obvious.
- 2
Trim the purpose list
Delete the options that do not apply so the dropdown answers one job rather than five.
- 3
Share the link directly
Send the link in the group chat or message thread where people are already discussing the event.
- 4
Chase from the export
Filter your guest list against the responses and message only the people still missing.
- 5
Delete when done
Remove the responses once the post has gone out; an old address list is a liability, not an asset.
Mistakes to avoid
- Asking for an address in a single free-text box, which produces missing postcodes and unusable rows.
- Collecting addresses without saying what they are for, which makes the request feel like data harvesting.
- Keeping the list forever — a card list from three years ago is mostly wrong and still your responsibility.
What it pairs with
Most teams don't run this form on its own. These are the forms and systems it sits next to.
What to do with the responses
Export to CSV, run it straight into your label or mail-merge tool, and delete the list once the invitations or parcels have gone out.
Works in both layouts

Frequently asked questions
Does it check that the address is real?
The address block enforces structure and required parts. For full postal validation, connect an address lookup service in the builder.
Can I use it for wedding invitations?
That is the leading use. Send the link with your save-the-date and collect every address before the invitations are printed.
What about international addresses?
The address block accepts international formats, including a country line, so overseas guests are not forced into a UK layout.
How long should I keep the list?
Only as long as the mailing needs it. Export, post, then delete — most people keep it for a single campaign or event.
