This form replaces the open-ended email with a short structured request a former manager can finish in five minutes: who they are, how they knew the candidate, the dates and role, a straight re-employment question, a conduct rating on a fixed scale, and one open box for anything the structure missed. Because every referee answers the same questions on the same scale, three references produce a picture rather than three anecdotes. Consent is recorded on the candidate's side before the request goes out, and the referee's employer and role are captured so the source can be verified. It differs from the Tenant Reference Request in what it is checking: a landlord reference is about rent paid and property kept, whereas this one is about conduct, reliability and whether the employer would take the person back. Two rules open a required explanation whenever an answer is anything other than a clean yes.
