About this template
Volunteer recruitment fails when willing people are asked vaguely and never contacted, and this form fixes both ends of that problem. It records the volunteer's contact details and how long they have been part of the congregation, then asks which teams they would like to join, from welcome and hospitality to worship, technical, children's work, youth, small groups, maintenance and community outreach. Availability is captured concretely — which services, which weeks of the month, and whether they can help midweek or at short notice — which is exactly what a rota coordinator needs. Skills and experience questions surface the musician, electrician or first aider hiding in the third row. Because some roles involve children or vulnerable adults, the form asks whether the volunteer has a current background check and whether they are willing to undergo one, and explains that references and safeguarding training are required before those roles begin. Two references are collected up front so the process is not held up later.

Frequently asked questions
- Does it enforce safeguarding steps?
- Choosing children's or youth ministry makes the background-check question required and the agreement text sets out what must happen first.
- Can team leaders be notified directly?
- Yes — route notifications by team so the worship leader and children's leader each receive their own volunteers.
- Can we build a rota from the answers?
- Availability answers export to CSV, giving your coordinator a ready-made grid of who can serve when.
- Can existing volunteers re-confirm each year?
- Send the same form annually as a renewal so availability and checks stay current.
