About this template

Nurseries carry a regulatory duty to hold complete records for every child, and this enrolment form is structured around what inspectors actually ask to see. It captures the child's full name, date of birth, preferred name, home language and any additional needs, then both parents' or carers' details with parental responsibility clearly identified. Two emergency contacts are collected because one is never enough. The medical section records the doctor's surgery, health visitor, immunisation status, allergies with severity and symptoms, medication with administration consent, dietary requirements and whether the child has an EpiPen or inhaler on site. Routine questions cover naps, comfort items, toileting stage, feeding and any words the child uses for the toilet, which is exactly the detail that makes a first week calmer. Sessions requested are captured by day and by session so the register can be built. Consents are split by purpose: photography, outings, sun cream, plasters, emergency treatment and nappy changing, each agreed separately and signed.
