About this template
Lash and brow work sits close to the eye, so a consultation form should behave like a screening tool rather than a booking note. This card-style form asks one question per screen: contact details, the treatment wanted, and whether the client has had extensions, lifts or tints before. Eye health questions cover conjunctivitis, styes, dry eye, recent eye surgery, contact lens use and known adhesive or dye sensitivities, with a required patch-test confirmation for tinting. Style goals are captured visually in words — natural, wispy, hybrid, volume, mega volume for lashes, or shape, thickness and mapping preference for brows — plus curl, length and colour where relevant. The aftercare screen sets expectations on water, oil-based products, mascara and infill timing, and the client confirms they understand that infills are needed every two to three weeks. Consent covers photography for your portfolio and your cancellation policy, and the whole thing takes under two minutes on a phone.
