About this template
Nail appointments vary enormously in length, and this form asks the handful of questions that let a technician block the right amount of time. The client gives contact details, then chooses the service — gel manicure, BIAR, acrylic set, infills, pedicure, soak-off or nail art — and says whether they are wearing existing enhancements that need removing, which is the most commonly forgotten twenty minutes in a nail diary. Shape, length and finish preferences are captured along with a colour or inspiration note and an optional reference photo. A short nail health section asks about lifting, breakages, fungal infection, recent injury and known allergies to acrylate or acetone, so the technician can advise before the client is in the chair. The final screen sets the aftercare and infill expectation, confirms the deposit and cancellation policy, and offers a reminder by text or email. Salons using it report far fewer overruns because removals and art time are known before the appointment starts.
