A Likert item is a statement plus a symmetric agreement scale, and most surveys that claim to use one get the details wrong. This template is built around the details. The scale is balanced — two negative points, a genuine neutral midpoint, two positive points — with the same wording on every item so respondents are not re-learning the scale each row. The items are grouped into short matrix blocks by theme, which reduces the fatigue that produces straight-lining, and two items in each block are deliberately reverse-worded so that a respondent ticking the same column all the way down becomes visible in the data. A 'not applicable' option is offered separately from the midpoint, because 'I have no opinion' and 'this does not apply to me' are different answers that a single neutral point conflates. One open question follows the batteries so respondents can say the thing the statements missed. The form also records how confident the respondent is in their answers, which is useful when you need to weight or explain an unexpected distribution.
