About this template
This is the workhorse instrument for primary quantitative research: one questionnaire that opens with a plain-language statement of what the study is for, records consent before a single answer is stored, and only then starts asking. The body follows the order that keeps completion rates up — easy, non-threatening closed questions first, the more demanding rating and open-ended items in the middle, and demographics at the end where people are already committed. Closed questions use fixed options so answers arrive as clean categories rather than free text, the rating items share one scale so they can be compared, and every sensitive question carries a 'prefer not to say' option so a non-answer is recorded honestly instead of guessed. Two open questions sit at the end for the things a fixed list cannot anticipate, which is usually where the most quotable material comes from. Conditional rules keep the form short: a follow-up appears only when someone reports a problem, and the contact block appears only for people who agree to be approached again. Everything lands as one row per respondent, which is the shape statistics software expects.
