About this template

Consultations only carry weight if the responses can be analysed and defended, and this survey is structured so the results can be reported as numbers rather than a pile of comments. It opens by explaining the proposal and the consultation period, then establishes the respondent's connection to the area — resident, worker, business owner, visitor or representative of a group — and their postcode district for geographic analysis. Core questions measure overall support on a five-point scale, then break the proposal into its components so you can see which elements drive opposition. Respondents rate impact on traffic, noise, green space, accessibility and local business, and rank their priorities. Open questions ask what would improve the proposal and what concerns have not been addressed, which is where the genuinely useful material appears. Optional equality monitoring questions are clearly marked as voluntary. Because the whole survey is one question per screen, completion rates on mobile are far higher than a long single-page form, and every answer exports cleanly for the consultation report.
