About this template
A skills assessment is only worth keeping if somebody could disagree with it on the evidence. This form is built to force that evidence into the record. The assessor names the person, the role profile being assessed against and whether this is a baseline, an in-role review or a promotion readiness check — a promotion check opens the extra questions about scope, autonomy and the level above. Each skill area is rated on a four-point scale from awareness to expert, and every rating that lands below the level the role requires opens a box for the specific example that justified it and a development action with an owner and a target date. Practical skills can carry an attachment — a work sample, a recording, a signed observation sheet — so the file travels with the score. At the end the assessor states an overall readiness position and whether a re-assessment is needed, which turns the document from an opinion into a plan that has a next date on it.
