This is the real form. Answer it to test the 2 conditional rules — nothing is sent or stored.
2 questions are hidden by conditional logic right now — change an answer above to reveal them.
Compliance checklist
- Payment items priced — passingEvery payment question charges a real amount, so a submission always records a payment.
- Conditional logic resolves — passingEvery rule points at a question or page that exists in this template.
- Legal text not required — passingThis template collects neither health nor regulated financial information.
- Question labels unique — passingLogic and exports address questions by label, so duplicates are ambiguous.
What happens after submit: nothing is stored in this preview. On your own copy, every response triggers an instant email notification and lands in your response dashboard, ready to export or forward.
Happy with it? Take a copy into your workspace — every question, rule and setting comes with it.
Who this template is for
Spanish Contact Form is built for sales teams, agencies and independent consultants who need to qualify inbound demand before anyone spends time on a call.
- Sales teams, agencies and independent consultants working in lead generation & sales.
- Teams who need to qualify inbound demand before anyone spends time on a call without writing code or paying for a custom build.
- Anyone replacing unqualified enquiries clogging the pipeline with one structured record per enquiry.
- Respondents are prospects arriving from ads, search and referrals — the form asks them 10 questions across 1 screen.
- Mobile-heavy audiences, thanks to the one-question-per-screen card layout.
Why this form is useful
It removes the cost of unqualified enquiries clogging the pipeline and turns each submission into a record your team can act on immediately.
- 5 questions are required, so submissions arrive complete instead of needing a follow-up email.
- It collects a verified email address so replies and confirmations land.
- It captures a phone number for urgent follow-up.
- It allows multiple selections without free text.
- Conditional logic hides 3 questions until they are relevant — shorter forms convert better than long ones.
- Every response is stored, searchable and exportable, so nothing depends on one person's inbox.
How to use this template
From copy to live form is a few minutes of work, and every step is reversible.
- 1Press “Use this template” — a fresh copy of Spanish Contact Form lands in your workspace, ready to edit.
- 2Rename or delete any question, and change what is required. Nothing here is fixed.
- 3Open Notifications and add the email addresses that should be alerted on each submission; add an auto-reply to the respondent if the form collects an email address.
- 4Publish it, then either share the link directly or paste the embed snippet into your site — the embedded form resizes to fit and loads no marketing or advertising trackers.
- 5Watch responses land in Submissions, where you can filter, label and export them to CSV or PDF.
Logic and conditions blueprint
Exactly how this form behaves as it is answered — 3 conditional rules ship with it.
- Features conditional validation: if “¿Cómo prefieres que te contactemos?” is Teléfono, “Teléfono” becomes mandatory before the form can be sent.
- Features advanced show logic: if “Tema de tu mensaje” is Ventas, the form dynamically exposes “Nombre de la empresa”. Otherwise that question never appears.
- Features advanced show logic: if “Tema de tu mensaje” is Soporte, the form dynamically exposes “Número de pedido o cuenta”. Otherwise that question never appears.
- All 3 rules are editable in the Logic tab, and the built-in checker warns about rules that contradict each other.
About this template
Most contact forms fail Spanish-speaking visitors in the same way: the page is translated but the form underneath still says "Full name" and "Message". This template starts from the other end. Every question, placeholder, help text and choice is already written in neutral Latin-American Spanish — the kind that reads naturally whether the sender is in Mexico City, Bogotá or Miami — so you can publish it as-is and get answers the same day. The structure is deliberately the same as the standard contact form so nothing about routing changes: name and email first, an optional phone, a topic dropdown that lets one form serve sales, support and general questions, and a single open message box. Conditional rules ask for a company name only on sales enquiries and an order number only on support requests, so a first-time visitor is never shown fields that do not apply. A separate consent checkbox covers marketing email, kept apart from the enquiry so it can never block a message. Pair it with the Spanish autoresponder and the person who writes to you gets their confirmation in the language they wrote in, which is the part people actually notice.
What this form asks

How the form changes as it's filled in
This template ships with 3 conditional rules, grouped into 3 behaviours driven by 2 questions. Everything below is already set up — edit or delete any rule once the template is in your workspace.
Driven by “¿Cómo prefieres que te contactemos?”
- Makes answers required
If “¿Cómo prefieres que te contactemos?” is Teléfono, “Teléfono” becomes required before the form can be submitted.
Driven by “Tema de tu mensaje”
- Reveals questions
If “Tema de tu mensaje” is Ventas, the form reveals “Nombre de la empresa”.
- Reveals questions
If “Tema de tu mensaje” is Soporte, the form reveals “Número de pedido o cuenta”.
Set it up step by step
- 1
Publish it without translating anything
The labels, placeholders and choices are already Spanish; change only your company name and the topic list.
- 2
Send the confirmation in Spanish too
Set the form language to Spanish so the autoresponder, validation messages and success screen all match the questions.
- 3
Keep the topic list to five options
Short lists get honest answers; long ones get whatever sits at the top of the menu.
- 4
Route by topic
Use the topic answer to notify the right inbox instead of publishing one form per department.
Mistakes to avoid
- Translating the page but leaving the form fields in English, which is the single fastest way to lose a Spanish-speaking lead.
- Making the phone number required, which drives away people who only ever wanted to write.
- Replying in English to someone who wrote in Spanish because the notification email gave no hint of the language.
What it pairs with
Most teams don't run this form on its own. These are the forms and systems it sits next to.
- Contact Us Form
- Customer Feedback Form
- Online Appointment Booking
What to do with the responses
Set the form language to Spanish, connect a monitored inbox and reply in the language the message arrived in — that single habit is what turns Spanish traffic into Spanish customers.
Works in both layouts

Frequently asked questions
How is this different from the Contact Us Form?
The structure is the same on purpose, but every question here is written in Spanish rather than translated after the fact, and the shipped conditional rules reference the Spanish labels so nothing breaks when you publish.
Will the confirmation email be in Spanish?
Yes, once you set the form language to Spanish: the autoresponder, validation messages and success screen all follow that setting, independent of the language you use in the builder.
Can I make it bilingual?
Publish this one for your Spanish audience and the Contact Us Form for English, then link them from each other — one form trying to hold two languages ends up harder to read in both.
Can I edit the questions?
Yes. Everything is a normal field, so you can rename, reorder or delete anything; if you rename a label used by a rule, update the rule to match.
Is the Spanish Contact Form template free to use?
Yes. You can preview and test Spanish Contact Form on this page without an account, and take a copy into your own HelloForms workspace on the free plan. There is nothing to install and no card required to publish it.
What does the Spanish Contact Form template ask for?
It asks 10 questions across 1 page, 5 of which are required. Every question is listed in full further down this page, and each one can be renamed, reordered, made optional or deleted after you copy the template.
Can I edit the Spanish Contact Form form after copying it?
Yes — the copy is entirely yours. Change wording, add or remove questions, switch between the classic and card layouts, restyle it to match your brand, and set who gets notified on each submission.
How does the conditional logic in this lead generation & sales form work?
3 conditional rules ship with the template: Features conditional validation: if “¿Cómo prefieres que te contactemos?” is Teléfono, “Teléfono” becomes mandatory before the form can be sent. Features advanced show logic: if “Tema de tu mensaje” is Ventas, the form dynamically exposes “Nombre de la empresa”. Otherwise that question never appears. Features advanced show logic: if “Tema de tu mensaje” is Soporte, the form dynamically exposes “Número de pedido o cuenta”. Otherwise that question never appears. All rules are editable in the Logic tab.
Where do responses to the Spanish Contact Form form go?
Submissions land in your workspace under Submissions, where you can search, filter, label and export them to CSV or PDF. You can also email a notification to your team on every submission and send the respondent an auto-reply.
Can I embed the Spanish Contact Form form on my own website?
Yes. Publish the form and paste the embed snippet into any page or share the direct link. The embedded form resizes to fit its container and loads no advertising or marketing trackers inside the iframe.
